Returning Student Information Form
Our School District is transitioning from paper to electronic records to save funds and time involved with completing the necessary paperwork for your child to return each year to school.
Therefore, as we did last school year, we will not be sending home the paper forms on the first day of school. Instead, the forms will be online for you to complete. It is important that you take the time to follow the directions, which contains a link to the form, on this page. Each student is required to have each of the forms completed yearly.
The Hermitage School District provides special services to students through Special Education, Gifted Education, and 504 medical accommodations. The goal of each of these areas is to help students find success in the school setting.
As a staff, we work diligently to meet students' unique educational needs. This work involves monitoring student progress in order to determine if each child is receiving the type and level of support that he or she requires. Through special services, such as special education or gifted education, we work to ensure that we are supporting each and every student in a way that helps them to find success.
