Returning Student Information Form Instructions

Welcome to the Returning Student Information Form Portal.

Please read the following information before entering the Portal.

A letter, with a 15 character Snapcode, was sent home for each student enrolled in 2018-19  This Snapcode is only valid for the 2019-20 school year, so you do not need to keep this code once you submit the form.  Once you enter the portal the student information we have on file will be filled in for you.  Please review this information carefully and make any corrections, where applicable.  If you have misplaced your snapcode, please send an email to This email address is being protected from spambots. You need JavaScript enabled to view it.with your student's name and grade and we can email the code to you or resend the letter via postal mail or email.

If you have an address change to submit, please contact the Student Registration Office at (724) 981-8750 extension 6000.

Depending on the grade your student is entering, you may be required to provide an updated Physical Packet, Dental Form or Immunization Records.  If your student is entering one of those grades and needs to provide this information, you will be prompted to do so.  If you have already provided that information to the school nurse, you do not need to do so again.  If you are unable to scan the necessary forms, please bring a printed copy to the Central Administration Office or to your school nurse.

When you click on the link below, you will be asked to enter your 15 character Snapcode.  You will then be asked to enter your account username and password, which is the same account used last year, or you will need to create an account if this is your first time in the portal.  Once your account is verified you will then be asked to enter the student's birthday to confirm the Snapcode you entered.
